Supported Home Care - This "in home" service option provides a personal assistant (PA) for a set number of hours each week to a child/adult living at home with their parents. This PA provides direct assistance in daily living. Supported Home Care is available in each of the 18 counties served by DSNWK.
Supported Living - Individuals who reside in their own home or apartment. These individuals need intermittent support to maintain a safe and healthy lifestyle and thereby achieve successful integration into their home communities. Supported Living is available in Atwood, Hays, Hill City and Oakley communities.
Supervised Living - DSNWK's Supervised Living program provides a variety of supports and services for those individuals requiring 24 hours of on site staff supervision. The options available include but are not limited to supporting those persons with behavioral challenges, those experiencing medical or aging complications and those with accessibility needs. DSNWK's supervised living homes are offered throughout the 18 county region and include potential residency within one of DSNWK's ICF/MR homes. The ICF/MR program is a specialized program funded by the Kansas Department of Aging which provides on site nursing support and oversight by the department of aging. Regardless of the location, DSNWK's supervised living program is committed to providing safe and healthy living environments with qualified and caring staff. Supervised Living is available in Atwood, Catherine, Hays, Hill City, Norton, Russell, and Victoria communities.
