Case Management

Service Coordination, also known as case management is available for eligible children, youth, and adults with Developmental Disabilities. This is a comprehensive service that is designed to enable individuals and/or their families to locate, utilize, and coordinate available resources, services and opportunities within their community of choice. Service Coordination is specifically tailored to each individual. The Individual chooses the service provider(s) within the options available and can elect to make changes as they fee appropriate. The service is billed through the person's medicaid card or through a private pay agreement based on a sliding fee scale. The Service Coordinators are assigned by geographic location within the 18 county service area to provide the most efficient service.

DSNWK Service Coordinators possess a Bachelor's degree in a Human Services field, must have 6 months experience working with people with developmental disabilities and must successfully complete Targeted Case Management training through the State of Kansas. The qualified staff are also responsible for following the Rules of Conduct for Case Managers serving people with Developmental Disabilities. In the event that there is a conflict of interest for any DSNWK Service Coordinator,an appropriate replacement or referral to another service provider will be made.

DSNWK Service Coordination believes that the process of identifying, developing and providing supports and services based on the individuals abilities, interests, goals, and needs are essential for success. This information is developed through the Life Planning process in which the individual and their team (support network) can address the person's preferred lifestyle. Other information in the life plan may include; medical, financial, emotional/relationships, behavioral, employment, religious, cultural and ethnic preferences. DSNWK also believes in continuing to find new systems to improve the efficiency and effectiveness of the Life Planning process. The Life Planning process also focuses on assuring quality services through the Quality Assurance reviews by the DSNWK/CDDO, Kansas Lifestyle Outcome reviews through the SRS program, on-site monitoring, consultation, specific training curriculum and a comprehensive evaluation process.
